The root cause has been traced to a core service update that was applied out of hours on 4th June. Some serverless functions in some clusters failed to update with the latest code and this caused some URL's for some customers to fail unexpectedly during the scan process. After identifying and updating the failed nodes, the issue was resolved (subject to DNS propagation). We apologise for the inconvenience and have updated our post-deployment checks to cater for this scenario going forward.
Engineers are investigating reports of some on-demand URLs failing to scan and displaying unexpected error messages.
